Earlier this week - CEO apologized for several delays on Monday and now this.

TTC officials are trying to find out exactly what happened when a train door opened while it was stopped inside a tunnel.

It happened at the height of rush - hour - 8:30 Thursday morning near King Station on the Southbound Yonge line.

The door incident was blamed on human error.
