The Ontario Government says human error at the Provincial Emergency Operations Centre, was the main reason a false alert was sent out regarding the Pickering Nuclear plant.
The alert was sent back on January 12th.
The report goes on to say that after the alert was sent out in error, "despite a series of consultations, no clear instructions were provided to the on-duty officers in the immediate aftermath of the first alert.
Supervisors were uncertain about whether or how to issue a second alert as broadcast intrusive, meaning that it would be broadcast in the same manner as the first alert."
Government officials say they have already taken corrective measures, "in key areas, including planning, procedures, operations, communications and staff training."
