Officials with the Toronto District School Board say 15-year old Jeremiah Perry should never have been allowed on the field trip to Algonquin Park on which he drowned because he failed a mandatory swimming test.
The teen died on July 4th.
The TDSB says Perry was not provided any additional swimming lessons after he did not pass the test but was allowed to participate in the field trip, anyway.
TDSB Education Director John Malloy says documents show that of 32 students who were on the trip, 30 students took the test and 15 of those students did not pass.
There are no record of the remaining 2 students ever taking the swimming test.
Malloy could not explain why Perry was allowed on the trip.
He stresses that the school board is looking into why safety procedures were not followed and adds that it will take steps to make sure that such a break with policy does not happen again.
The Director adds that the two teachers who organized the field trip are "on home assignment" and have both refused to speak with investigators.
Perry had been swimming with his classmates in Big Trout Lake while on a camping trip when he died.
Witnesses say he slipped under the water and didn't come back to the surface.
He was not wearing a life jacket.
Perry's body was found the next day.
The OPP and the Coroner's Officer are investigating.
